This document is a mandatory form for all students who attend any class at ECC Dance Co. In case of emergency, it is important that our teachers can contact any students parents/guardian as quickly and easily as possible. Please complete and hand to your class teacher.

MEDIA RELEASE

Before ECC Dance Co releases any of the following: rehearsal/performance footage, photography, class videos or media content, we require all students (alternatively a parent/guardian if under 18) to complete a Media Release Form to authorize the use of such content. Various visuals and social media platforms are powerful methods to help promote and advertise. However, we are only happy to do so with your authorization.

ECC CLOTHING

Please complete a Uniform Order Form at the beginning of each term to place your order for any ECC Dance Co merchandise. Ensure all order deadlines are met and handed to your class teacher.

CLOTHING/FOOTWEAR SIZING FORM

PURPOSES ONLY

Please ensure each student has completed a sizing form upon attendance to their first class and/or rehearsal in respect to all additional clothing and costumes that is not uniform. Additional clothing could include: Elite Competition Teams costumes or performance attire.
